The mission of Gonzaga University's Student Outreach and Support form–is to support cura personalis—care for the whole person. As a staff or faculty member, you may encounter students who need your assistance for academic reasons, physical and mental health reasons, and other important reasons. You may be the first contact and play an important role at GU in providing resources to support students.
The Student Outreach and Support form allows staff and faculty to inform the appropriate offices about students who need support and advocacy. These forms are used to initiate collaborative action plans to assist the referred student.
- If you are concerned about the student’s attendance and/or absences from class, have you attempted to contact the student? If “No”, please attempt to contact the student. If the student does not respond or the response is concerning, please submit this form.
- Once you submit a report, the report is received by the Center for Cura Personalis. Depending on the reason(s) of concern you indicate in this report, additional student support teams are automatically contacted to assist the student. Additional teams notified to support this student may include:
All reports are monitored during university business days/hours (Monday-Friday, 8:00 am – 5:00 pm)
Please use this form to report behaviors that may cause concern but does not constitute an emergency.
- For Emergencies: Your initial response will depend on the urgency of the situation. For all police/fire/medical emergencies which are life or property threatening you should immediately call 911. After you have given the information to the 911 operator, immediately call Security and relay the same information (call 509-313-2222). If there is no immediate threat, call 509-313-2222 and advise the dispatcher of your circumstances. An officer will be dispatched to make contact with you.
- While a student may be referred to counseling services and/or the health clinic as part of this process, this process does not replace appointment scheduling. A student can make an appointment with Health Clinic and Counseling Services at any time.
